First Visit
Brief Paperwork
Prior to your arrival at our office, you will receive a text message with a link to fill out our digital intake forms. If you are not able to access it digitally, we also have paper copies in the office.
Note, if you require a paper copy of the intake form we ask that you please arrive 10 minutes early to complete before your appointment.
Consultation
Once the paperwork has been completed, you will have a consultation with one of our staff to discuss your health-related problems, concerns, as well as potential treatment options.
History and Examination
In order to determine what your actual problem is, the doctor will ask you various questions related to your condition. Next, a number of specialized tests will be performed to determine which tissues are affected and to what extent.
X-ray Studies
Your specific condition may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you.
Home Instructions
At the conclusion of your initial visit, you will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.
Schedule Your Next Appointment
It is imperative that the Doctor be given time to review your exam and x-ray findings prior to rendering any treatment. For this reason, you will be given a convenient appointment time for your follow-up visit. Under special circumstances, some therapies may be administered on the intital visit in order to ease those in severe pain. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of findings. This review of findings will be performed at no cost. Treatment options for your condition and all finances will be discussed during this time. If care is accepted, treatment will begin right away.
